Undergraduate Programs

Incoming First Year

  • Photocopy of Form 138 (Senior High School Report Card)
  • Photocopy of PSA Birth Certificate
  • 2 pcs 2×2 picture, white background with nametag
  • Fill out and submit admission form online at https://basc.priisms.online

Transferee

  • Photocopy of Honorable Dismissal and Certificate of Grades
  • Photocopy of PSA Birth Certificate
  • 2 pcs 2×2 picture, white background with nametag
  • Fill out and submit admission form online at https://basc.priisms.online
Note: After passing the BASC Admission Test (BASCAT), students should accomplish the Enrollment Requirements and follow the enrollment guide posted at the BASC Office of the Registrar Official Facebook Page.

Enrollment Requirements for Incoming First Year BASCAT Passers

  • Original Copy of Report Card (Form 138)
  • Photocopy of PSA Birth Certificate
  • 2 pcs 2×2 picture, white background with nametag
  • Completely filled out Enrollment Form (can be downloaded at [link to be provided])
  • BASC Student ID No (will be generated at https://basc.priisms.online after passing the BASCAT)

Enrollment Requirements for Transferees BASCAT Passers

Reminder: All original documents will be submitted only to the Registrar's Office upon Enrollment. Students should keep a photocopy of their Cards for scholarship purposes.
